Features
- There are seven field types: text, paragraph, checkbox, radio button, email, dropdown, and date.
- You can add a form title and description.
- You can set the field position, label, label position, placeholder, enable or disable the field, make it required, add help text, choose full/half/one-third size, and add a custom field class.
- Input fields can be made sortable.
- You can customize the title, button, description, and input field color, typography, padding, and border.
- You can set the success message color and typography.
How to use
- First, install the Survey Form plugin
- Add the Survey Form block from the block category called “Widgets” in the Gutenberg editor.
- You can change block settings from the right-side settings sidebar.

From Gutenberg Editor:
- Go to the WordPress Block/Gutenberg Editor
- Search For Survey Form Block
- Click on the Survey Form Block to add the block
Download & Upload:
- Download the Survey Form Block plugin (.zip file)
- In your admin area, go to the Plugins menu and click on Add New
- Click on Upload Plugin and choose the
survey-form-block.zipfile and click on Install Now - Activate the plugin and Enjoy!
Manually:
- Download and upload the Survey Form Block plugin to the
/wp-content/plugins/directory - Activate the plugin through the Plugins menu in WordPress
